Name of Form Purpose of Form California Dream Act Application 2022-23 Available October 1, 2021. This form is used to apply for financial aid for the 2022-23 academic year by students who qualify for an AB 540 nonresident tuition exemption. Ineligible PLUS Borrower Form 2022-23 Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment. Do not use this form if your credit was denied; see the options if credit is denied section (#9) on the PLUS Application Tips page. Please have each parent listed on the FAFSA submit this form. Free Application for Federal Student Aid (FAFSA) 2022-2023 Available October 1, 2021. This form is used to apply for financial aid for the 2022-23 academic year.

You can submit a Parent Contribution Appeal or a Student Contribution Appeal - we will review your appeal, make adjustments (as appropriate) based on your most current financial situation.

Native American Opportunity Plan Use this form if you need to provide supporting documentation to verify eligibility for Native American Opportunity Plan funding.

Free Application for Federal Student Aid (FAFSA) 2021-2022 This form is used to apply for financial aid for the 2021-2022 academic year. While the Department of Education and the University look to the family as the first resource for financing the education of a dependent student (as defined by the Department of Education), providing parental information on the aid application does not obligate parents to provide financial support. If at all possible, we encourage families to work toward providing all requested information so students can secure the best possible financial aid package. Summer Childcare Expense Appeal You may submit this appeal if you would like to request an adjustment to your Summer Cost of Attendance for childcare expenses. Log into the Financial Aid Forms portal and click on the Manage Requests button, then the + sign next to the Summer Child Care Appeal option. Be sure to upload documents to support your expense with your completed form. Summer Intercampus Financial Aid Application This form is for students taking Summer courses through another UC campus. You must complete this application in order to receive financial aid for your Summer intercampus enrollment.
