Name of Form Purpose of Form California Dream Act Application 2024-25 This form is used to apply for financial aid for the 2024-25 academic year by students who qualify for an AB 540 nonresident tuition exemption. Ineligible PLUS Borrower Form 2024-2025 Parent(s) listed on the FAFSA can submit this form if they were unable to complete the online PLUS application because of their non-citizen status, a federal loan default, or a grant overpayment. Do not use this form if your credit was denied. Please have each parent listed on the FAFSA submit this form.
Free Application for Federal Student Aid (FAFSA) 2024-2025 This form is used to apply for financial aid for the 2024-25 academic year. We will review your appeal, make adjustments (as appropriate) based on your most current financial situation.

Native American Opportunity Plan Graduate students - completed the Native American Opportunity Form to provide supporting documentation to verify eligibility for Native American Opportunity Plan.
Undergraduate students can confirm eligibility the Native American Opportunity Plan by logging into the Berkeley Financial Aid Forms website. If the form has not yet been assigned to you, after logging in, click on the Manage Requests button at the top right, and click the + to open the form.
